Professional billing support

Out-of-Network Medical Billing

Konnext helps practices prepare and submit out-of-network (OON) professional claims and follow up with payers. We define the agreed work and handoffs before billing activity begins; other tasks require a separate agreement.

What out-of-network billing means

On this page, out-of-network describes a professional billing workflow where the provider is outside the applicable health plan network. Network status needs to be checked for the provider, service location and plan product. Preparing or submitting a claim does not establish participation, confirm benefits or authorization, or guarantee payment.

Konnext's service covers the administrative tasks agreed with your practice: claim preparation, submission and payer follow-up. The practice retains responsibility for clinical records and decisions. Any other work needs a separate agreement.

Nationwide support for professional practices

Konnext provides out-of-network professional billing support across the United States for the professional-practice specialties we serve. Each engagement covers the agreed claim preparation, submission and payer follow-up for the specific providers, locations, payer products and services. This scope does not include facility billing.

The work included in your agreed scope

Claim preparation

Prepare professional claims from the practice's approved billing information and available documentation. Identify missing information for the practice to resolve rather than creating clinical facts or assuming an unresolved detail is complete.

Claim submission

Submit the agreed claims through the practice's approved billing channel, using the applicable payer instructions. Keep the submission result available for follow-up; a transmission or acknowledgement is not a payment decision.

Payer follow-up

Check claim status, record payer responses and identify the next administrative action. Requests requiring another task or a practice decision are handed back with the open issue identified.

How the work is organized

  1. 01

    Confirm the assignment

Identify the practice, professional billing need, payers, service locations and work to be handled. Agree the three-task scope, required access and responsibilities before work starts.

  2. 02

    Prepare the claim information

Organize the approved billing information and identify missing records or decisions that need the practice's attention.

  3. 03

    Submit through the agreed channel

Use the appropriate payer instructions and the practice's approved system. Preserve the available submission response for follow-up.

  4. 04

Follow up and hand off exceptions

Record the payer status and next action. Keep unresolved practice dependencies and requests outside the agreement separate from the included work.

Network status and program rules need separate review

An out-of-network label is not a universal billing permission. Commercial plan networks, Original Medicare participation and opt-out status, Medicare Advantage arrangements and Medicaid program requirements are different contexts. The applicable program, payer product, provider and service must be identified before the practice relies on a billing pathway.

This page describes administrative billing support. It does not determine whether a patient may be billed a particular amount or whether a claim qualifies for an appeal or payment dispute.

Out-of-network billing FAQs

What is included in Konnext's out-of-network medical billing service?

Konnext's professional billing service covers claim preparation, submission and payer follow-up. Other tasks require a separate agreement. The engagement identifies the work assigned to Konnext and the responsibilities retained by the practice.

Does submitting an out-of-network claim make us an in-network provider?

No. Claim submission does not establish payer participation or an in-network effective date. Network status must be checked for the provider, location and plan product; it is separate from the administrative claim-submission task.

Does this service include eligibility checks, authorization, coding or patient billing?

Those tasks are not automatically included in this out-of-network service. Any additional work requires a separate agreement defining availability, responsibilities and boundaries.

Are appeals, negotiations and payment disputes included?

No. They are outside the claim preparation, submission and payer-follow-up scope. Do not assume that a follow-up service includes an appeal, negotiation, arbitration, Independent Dispute Resolution or legal representation.

Does out-of-network billing guarantee coverage or payment?

No. Claim submission does not confirm plan benefits, authorization, coverage or payment. The applicable payer and program requirements and the circumstances of the service still matter.

What should we share in an initial inquiry?

Share high-level business information such as practice type, professional billing need, service states, payer mix and current system. Do not submit patient information, claim details, medical records or access credentials through the public website. Detailed information is handled only through an agreed secure workflow.

Discuss the billing work your practice needs

Start with a business inquiry about your out-of-network billing needs. On the inquiry form, you can choose “Medical billing or RCM” and mention out-of-network billing in your message. Our team will contact you to discuss fit and next steps. Do not send patient records, claim details or access credentials.
